Job summary

Lincolnshire County Council in Louth is recruiting a Community Care Officer to support across NLAG hospitals and help people leave hospital safely and return home. You will be part of a busy multidisciplinary team, assessing needs, planning care, and coordinating with health and social care colleagues to enable independent living.

This 37-hour permanent role is based at Louth, with travel to multiple NLAG sites as required, and involves close collaboration with hospital staff to ensure smooth

Qualifications

• Experience using strengths-based approaches to support independence.
• Ability to identify needs, develop solutions and support outcomes.
• Experience working with families and professional networks.
• Knowledge of health and social care legislation.
• Experience with risk assessment and safeguarding.
• Excellent IT skills and clear record-keeping.
• Understanding of financial assessment and welfare benefits is helpful.

Responsibilities

• Completing assessments and identifying care and support needs.
• Working in a strengths-based way to achieve positive outcomes.
• Helping to arrange and develop personalised care and support packages.
• Providing information and advice to people and their families.
• Working with health, housing, voluntary and independent sector colleagues.
• Supporting qualified social workers, therapists and nurses.
• Identifying and managing risk, including safeguarding concerns.
• Arranging equipment and minor adaptations where appropriate.
• Monitoring and reviewing care and support arrangements.
• Contributing to multidisciplinary and multi-agency meetings.
• Keeping accurate records and using information to monitor progress and performance.
