Job description

Are you looking for a role where no two days are the same?

Our Hospital Social Care Team at Lincoln County Hospital is looking for a Community Care Officer to join us to support across the Northern Lincolnshire and Goole (NLAG) hospitals, including Grimsby Hospital, Scunthorpe General Hospital, Goole and District Hospital, Diana, Princess of Wales Hospital and Hull Hospital. The post is based at Louth Hospital.

What will you be doing?

You will be part of our Lincoln County Hospital Team, providing social work support across the Northern Lincolnshire and Goole (NLAG) hospitals, including Grimsby Hospital, Scunthorpe General Hospital, Goole and District Hospital, Diana, Princess of Wales Hospital and Hull Hospital. Whilst covering these settings, your base will be Louth. You will be part of a busy, supportive multidisciplinary team, working alongside social workers, occupational therapists, nurses, therapists and health colleagues to help people leave hospital safely and return to living as independently as possible.

Hospital-based, but not limited to one hospital

Although you will be part of the team at Lincoln County Hospital, our team also supports hospital discharges from Northern Lincolnshire and Goole NHS Foundation Trust (NLAG) hospitals, including Grimsby Hospital, Scunthorpe General Hospital, Goole and District Hospital and Diana, Princess of Wales Hospital. This means that travel to other hospital sites may be required as part of the role.

You will have the opportunity to work across health and social care, building strong professional relationships and helping people move from hospital back into their communities.

When will you work?

This is a 37-hour permanent position.

Our usual working hours are:

Monday to Thursday: 8.45am to 5.15pm

Friday: 8.45am to 4.45pm

The team also works weekends and bank holidays on a rota basis.

What are we looking for?

You will need experience of working in a strengths-based way and supporting people to maintain their independence. You will need to understand how to identify needs, develop solutions and work with people and their families to achieve agreed outcomes.

You will also need:

  • Experience of working with families and professional networks.
  • Knowledge of legislation relevant to health and social care.
  • An understanding of risk and how to manage identified risks.
  • Good communication and interpersonal skills.
  • The ability to manage competing priorities under guidance.
  • Experience of working effectively with other agencies and professionals.
  • Good IT skills and the ability to record information clearly.
  • Knowledge of financial assessment, charging processes and welfare benefits would be beneficial.

The role requires a good standard of general education, including English and Maths, with a minimum of four GCSEs at Grade C or above or equivalent.

Why join our hospital team?

This is a great opportunity if you enjoy working with people, solving problems and being part of a wider multidisciplinary team.

You will play an important part in helping people achieve the best possible outcomes from their hospital stay, while supporting them to return home or move on to the right care and support.

If you are someone who enjoys a busy environment, likes working with different professionals and wants to make a practical difference to people's lives, we would like to hear from you.
