Job summary

Lincolnshire Community Health Services NHS Trust is seeking a Registered Nurse to join the Louth and Wolds District Nursing Team. The role involves autonomous practice delivering nursing care to patients in their homes, care homes and clinics, with 12-week induction and robust preceptorship.

Candidates will assess needs, plan, deliver and evaluate care, including wound management, continence, palliative care, medication administration and long-term condition management.

Qualifications

• Registered Nurse with NMC registration required.
• Ability to assess care needs and plan/ deliver/evaluate nursing care.
• Experience in community/home-based care preferred.

Responsibilities

• Assess care needs and plan, deliver, and evaluate nursing care.
• Provide wound management, continence care, and palliative care in patients' homes or clinics.
• Administer medications and manage long-term conditions.
• Work with MDT to coordinate patient care across settings.
