Job summary

Care Connect Group is seeking a Rapid Responder to provide urgent, person-centred care across community locations. Job Description – Rapid Responder

Care Connect Group are recruiting for the role of multiple Rapid Responders.

Job Title: Rapid Response Support Worker
Location: Various Community Locations
Reports To: Care Manager / Operations Manager

Job Purpose

The Rapid Responder provides urgent, person-centred care and support to individuals within the community who require immediate assistance due to hospital discharge, deterioration in health, unexpected absence of a regular carer, or other urgent care needs. The role aims to promote independence, prevent unnecessary hospital admissions, and support individuals to remain safely in their own homes.

The Rapid Responder will respond quickly to referrals, deliver high-quality care, and work collaboratively with healthcare professionals, families, and other agencies.

Key Responsibilities
  • Respond promptly to urgent care referrals across the service area.
  • Deliver high-quality personal care in accordance with individual care plans.
  • Assist with personal hygiene, dressing, toileting, continence care, and mobility.
  • Support service users with nutrition, hydration, and meal preparation.
  • Prompting with medication where authorised.
  • Complete welfare checks and monitor changes in physical or mental wellbeing.
  • Report concerns immediately using safeguarding and escalation procedures.
  • Support individuals following hospital discharge or during health crises.
  • Provide reassurance and emotional support to service users and families.
  • Maintain accurate electronic care records and incident reports.
  • Liaise effectively with nurses, social workers, GPs, therapists, and emergency services where appropriate.
  • Promote dignity, privacy, choice, independence, and person-centred care at all times.
  • Follow infection prevention, moving and handling, and health and safety procedures.
  • Participate in on-call rotas, evenings, weekends, and bank holidays as required.
  • Travel between service users throughout the designated geographical area.

Person Specification
Essential
  • Experience working in health and social care.
  • Up to date Patient, Moving and Handling
  • Compassionate, caring, and professional approach.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and make appropriate decisions under pressure.
  • Flexible approach to working hours.
  • Full driving licence and access to a reliable vehicle (unless otherwise agreed).
  • Ability to use electronic care recording systems.
  • Understanding of safeguarding adults and person-centred care.
Desirable
  • Level 2 or Level 3 Diploma in Health & Social Care.
  • Experience in domiciliary care, rapid response, reablement, or community care.
  • Medication administration training.
  • Moving and handling certification.
  • Knowledge of hospital discharge pathways.
