Job summary

Focus Ireland is recruiting a Health and Inclusion Project Worker for George's Hill Long-Term Accommodation in Dublin. The role delivers stability, specialist support and access to primary care and health services to residents at risk of homelessness.

You will conduct comprehensive health assessments, develop care plans, and work within a multidisciplinary team to promote wellbeing and independence.

Qualifications

• A degree or CORU registration in social care or related field.
• Experience with homelessness or vulnerable groups.
• Experience in team working and liaising with voluntary/statutory bodies.
• Strong report writing abilities.

Responsibilities

• Provide stability and specialist support for individuals based on health and addiction needs.
• Complete comprehensive assessments and develop individual support plans.
• Maintain professional relationships with customers and partners, ensuring dignity and self-determination.
• Support customers to access health, mental health and substance use services.
