Hilltop Community Resources seeks a Memory Care Specialist to deliver compassionate care in a supportive community setting in Grand Junction, CO. You will provide person-centered assistance, adhere to care plans, and monitor safety and wellbeing for residents with memory care needs.

The role emphasizes memory care skills, compliance with regulations, and collaboration with a team to enhance resident quality of life.

Qualifications

• GED or diploma with Alzheimer’s care exposure preferred.
• ADL assistance experience required or preferred.
• Minimum age 18 with QMAP certification for QMAP role.

Responsibilities

• Maintain professional boundaries and resident dignity.
• Follow Hilltop policies and state/federal rules.
• Provide protective oversight and safe living environment.
• Assist with transportation, events, and socialization.
• Ensure sanitation, safety, and housekeeping duties.
• Document care according to care plans and reports.
