Job summary

Corlann is hiring a Community Access Facilitator for our Waterford services on a permanent part-time basis. The role works with the Day Services team and multi-disciplinary colleagues to support people with intellectual disabilities to lead fulfilling lives.

You will promote person-centred planning, advocacy and opportunities in education, work and community life, while liaising with the circle of support to remove barriers and enable individuals to speak up for themselves.

Qualifications

• Possess a qualification in General Health and Social Care or Community and/or Youth Work (QQI Level 5).
• Or possess a relevant Health Care qualification at not less than QQI Level 5.
• Or an equivalent Health Care qualification from another jurisdiction.

Responsibilities

• Support people within the framework of New Directions and in collaboration with the person supported and their circle of support to co-produce person-centred plans.
• Lead and work with partners to access mainstream opportunities in education, work and civic life, promoting advocacy and self-direction.
• Ensure the person’s voice is central to planning and decision-making and help overcome barriers.
