Job summary

Focus Ireland in Limerick is seeking a Full Time, 1 Year Fixed Term Housing First Case Manager to deliver a Housing First approach to individuals experiencing long-term homelessness in Limerick City.

You will work with a multidisciplinary team, complete assessments and care plans, support access to housing and welfare rights, and maintain records using Focus Ireland systems. The role requires a degree in social care or CORU registration and strong team-working abilities.

Qualifications

• Requires a degree in social care or CORU registration.
• Experience with homelessness and dual diagnosis is desirable.
• Experience of working in a team and liaising with other agencies.
• Good report writing abilities and knowledge of housing services.

Responsibilities

• Deliver Housing First approach to individuals experiencing long-term homelessness in Limerick City.
• Provide a safe, welcoming and supportive service for clients.
• Maintain professional relationships and support clients to achieve stable housing and independent living.
• Complete assessments, care plans and support plans using Housing First systems.
• Assist clients to access and maintain housing, welfare entitlements and move-on accommodation.
• Support tenants to sustain tenancies and develop life skills.
• Develop links with social housing providers, landlords and other services.
• Advocate for clients at health, legal and social appointments.
• Work as part of a multidisciplinary team and with partner agencies.
• Provide information and guidance to prevent homelessness and support education, training and employment.
• Maintain up-to-date records using Focus Ireland PASS.
• Attend supervision and team meetings and contribute to service development.

Contract: Full Time, 1 Year Fixed Term

The purpose of this role is to work as a part of Housing First Case Management Service to deliver a ‘Housing First’ approach to individuals experiencing long term homelessness in Limerick City.

What You Will Do

• Work in line with the principles and fidelity of the Housing First model, promoting client choice, independence and self-determination.
• Provide a safe, welcoming and supportive service for individuals who are homeless, at risk of homelessness or living in temporary accommodation.
• Build and maintain trusting, professional relationships with clients, supporting them to achieve greater stability and sustain independent living.
• Complete assessments, care plans and support plans using Housing First case management systems and outcome measurement tools.
• Support clients to access and maintain suitable housing, welfare rights, entitlements and move-on accommodation opportunities.
• Assist tenants to establish and sustain successful tenancies, including developing life skills, managing rent payments, reporting repairs and being a good neighbour.
• Maintain and develop strong links with social housing providers, private landlords, homelessness services and other accommodation providers.
• Accompany and advocate for clients at appointments with healthcare, counselling, legal, social and other support services where appropriate.
• Work collaboratively as part of a multidisciplinary team and in partnership with statutory, voluntary and community agencies.
• Provide advice, information and guidance to prevent homelessness and support clients in accessing education, training, employment and community-based opportunities.
• Manage an active caseload, ensuring accurate and timely case notes, assessments, daily records and database updates using Focus Ireland systems and PASS.
• Attend supervision sessions, team meetings, handovers, training and external meetings, while contributing to service development and best practice standards.
• Work flexibly to meet service needs, maintain health and safety standards, support volunteers or students when required, and undertake additional duties as reasonably assigned.

What You Will Need

• A Third Level qualification at degree level in the area of social care i.e. youth work, social studies, addiction, psychology, etc and/or be a CORU registered Social Care Worker
• Experience of working with customers are or have been homeless
• Experience of working with customers identified as having a dual diagnosis
• Experience of team working
• Report writing abilities
• Experience of working and liaising with other agencies
• Knowledge and understanding of homelessness and issues that affect people who are or have been homeless (Desirable)
• Full driver’s license (Desirable)
