We seek a Basic Grade Social Worker to join a progressive team and contribute to patient-centered care within a compliant, safety-focused environment.

The role involves assessments, advocacy, discharge planning, and collaboration with multidisciplinary teams to support patient welfare and safety.

Qualifications

• Qualified Social Worker with CORU registration.
• 1 year's experience within a similar mental health setting.
• Effective verbal and written communication and IT skills.
• Good aptitude for learning.
• Good problem-solving skills.
• Good planning and decision-making skills.
• Ability to work to deadlines under pressure.
• Empathise with and understand the needs of others.
• Reliability and flexibility.

Responsibilities

• Provide individual social work assessments as identified, ensuring patient strengths are identified as part of care planning.
• Advocate for the patient and ensure advocacy services are available.
• Coordinate discharge and after-care arrangements with multidisciplinary teams and community services.
• Liaise with other agencies to ensure public protection and safety.
• Adhere to Mental Health Act 2001, Mental Health Amendment Act 2018 and all relevant regulations and policies.
• Follow GDPR and data protection guidelines in handling patient information.
• Perform other duties as appropriate to the post.

Job Objectives

We are seeking applications for the role of Basic Grade Social Worker. The role will suit experienced candidates who wish to progress in their careers in a progressive organisation and are deemed suitably qualified, skilled within a similar healthcare setting. To be familiar with and to adhere to all company policies and procedures. To use company IT systems as instructed and to never share your passwords with another or do anything likely to result in the unutilised disclosure of and or personal data as defined under the General Data Protects Regulations (GDPR). To take an active part in research projects relevant to secure care mental health nursing practice. To take all possible precautions to safeguard the welfare and safety of staff, patients, visitors and the public by implementing all policies and procedures relating to security- procedural, relational, and physical. To provide individual, social work assessments as identified, ensuring that the patient's strengths are identified as part of the overall treatment and care planning process. To ensure that any issues, where appropriate are addressed while the patient is receiving care and treatment in the facility. Together with another member of the multidisciplinary team liaise closely with Community teams and other agencies and ensure appropriate discharge and after-care arrangements while at the same time ensuring public protection. To act as an advocate for the patient, ensuring that the patient's views are heard and that advocacy services are available to all patients.
