Job summary

Barróg Healthcare is seeking an Aftercare Coordinator for the Cork office. You will lead an aftercare team, coordinate multi‑agency work, and develop and evaluate aftercare plans to support young people and vulnerable adults.

The role includes supervision, safeguarding duties, and involvement in ongoing service improvement. On‑site, full‑time position with a dedicated on‑call component.

Qualifications

• Fluent English language written and spoken.
• Eligibility to work in Ireland.
• Complete Level 7 qualification in Social Care or related field.
• CORU registration as a Social Care Worker.
• Management or coordinator experience in social care/health or disability would be advantageous.
• Full driver’s licence and willingness to travel between locations.

Responsibilities

• Lead and motivate a team of social care professionals supporting young people.
• Provide supervision for the Aftercare team.
• Interagency communications and keep referral agency updated.
• Direct support for clients when needed.
• Ensure involvement of young people in decision‑making and promote their voices.
• Uphold dignity, humanity and value in all practices.
• Support management with tasks and projects as needed.
• Conduct Health & Safety audits and property checks.
• Monitor timekeeping and attendance and ensure logs are compliant.
• Ensure professional standards across properties and contracts.
• Lead team meetings and coaching sessions.
• Support delivery of aftercare plans and cover operational shifts.
