Job summary

Focus Ireland is seeking an Aftercare worker to join the team delivering flexible housing support for young people in Clare. You will provide holistic casework, develop tailored care plans, and support residents toward independent living.

The role requires a social care degree or CORU registration, and a clean driving license for travel. You will collaborate with the team, liaise with external agencies, and contribute to records, supervision, and project evaluation while ensuring adherence to

Qualifications

A Third Level qualification in social care (degree level) or CORU registration as a Social Care Worker.
Experience of key-working clients or individual case-work.
Experience of managing challenging behaviour.
Full clean driver's license and access to own car.

Responsibilities

Work within Focus Ireland’s framework and model of care.
Deliver high-quality services as part of a team in Aftercare housing.
Develop and implement individual care plans for residents’ independent living.
Provide holistic support addressing self-care, health, budgeting, and living skills.
Promote confidence and self-esteem as residents transition from care.
Advocate on housing, welfare, education, and legal rights.
Liaise with external agencies and maintain inter-agency relationships.
Provide keyworking and assist in group work sessions.
