Job summary

Siloam Hospitals Purwakarta seeks a licensed pharmacist to deliver comprehensive pharmaceutical care for outpatients, review prescriptions, dispense medicines, and ensure safe medication use.

You will counsel patients, collaborate with doctors and nurses, maintain accurate records, and participate in quality improvement and training to uphold patient safety and service excellence.

Qualifications

• Bachelor’s degree in Pharmacy and active STR (Pharmacist) registration.
• Experience in prescription review, dispensing, and ensuring medication safety.
• Ability to counsel patients and work with doctors, nurses, and other teams.

Responsibilities

• Provide comprehensive pharmaceutical care for outpatient patients per hospital standards.
• Prepare, dispense, and ensure availability of medicines with proper labeling and inventory control.
• Counsel patients on medication use, dosing, storage, and adherence.
• Collaborate with doctors and nurses to optimize therapy and ensure continuity of care.
• Maintain accurate pharmaceutical documentation and participate in safety and quality initiatives.

Job Description

• Provide comprehensive pharmaceutical care and services for outpatient patients in accordance with hospital standards, pharmacy practice standards, applicable regulations, and patient safety requirements, including prescription review and identification of medication-related issues.
• Prepare, dispense, and ensure the availability of medicines and pharmaceutical supplies accurately and safely, including proper medication preparation, labeling, storage, inventory monitoring, and expiry date control.
• Provide medication counseling and education to patients regarding medication use, dosage, administration instructions, potential side effects, storage, adherence, and other relevant information to promote safe and effective medication therapy.
• Collaborate with doctors, nurses, and other multidisciplinary healthcare teams in optimizing medication therapy, resolving prescription or medication-related issues, and ensuring continuity and quality of patient care.
• Maintain accurate pharmaceutical documentation and reporting, implement medication safety and infection prevention practices, and participate in quality improvement initiatives, mandatory training, competency development, and continuing professional education.

Qualifications

• Minimum Bachelor’s Degree in Pharmacy (S.Farm.) and Pharmacist Professional Program (Apt.)
• Must hold an active Pharmacist Registration Certificate (STR)
• Have good knowledge and competency in prescription review, medication dispensing, pharmaceutical care, medication counseling, and medication safety
