Qualifications

• Diploma (D3) in Medical Records and Health Information.
• Experience with EHR systems, data entry and maintaining patient records is valuable.
• Strong attention to detail for accurate data entry and record maintenance.
• Excellent organizational skills for managing large volumes of medical records and ensuring efficient retrieval and storage.
• Ability to handle sensitive patient information with discretion and maintain confidentiality.

Responsibilities

• Compile medical record files and coding of diagnoses to support reporting.
• Arrange patient medical record files according to a predetermined structure.
• Classify disease diagnoses based on ICD-10 and ICD-9CM.
• Input disease codes into HIS or INA CBG systems for department access.
• Ensure completeness of forms for quantitative analysis of medical records.
• Process statistical data for daily, monthly, annual and incidental reports.
• Check completeness of primary data before, during and after treatment for history and statistics.
• Implement quality and safety procedures including training, data collection and reporting.
