Job summary

Christian Family Service Centre in Kwun Tong District is seeking an experienced Registered Nurse to lead a project supporting older people in maintaining intrinsic capability. The role requires clinical oversight, collaboration with a multidisciplinary team, and coordination with university researchers to ensure adherence to study protocols.

You will supervise staff, manage data in care platforms, and engage with community healthcare providers to drive high-quality elder care services.

Qualifications

• Registered Nurse with a valid Practising Certificate by the Nursing Council of HK.
• Experience in elderly care, community health, or integrated care services preferred.
• Demonstrated digital literacy and data management capabilities.

Responsibilities

• Lead planning, implementation, monitoring, and evaluation of a project for older people to develop or maintain intrinsic capability.
• Supervise project staff and coordinate multidisciplinary teams across elderly service units.
• Provide clinical oversight for screening, assessment, intervention, and care management per WHO ICOPE protocols.
• Monitor quality and consistency of screenings/assessments by volunteers and professionals.
• Coordinate with university research team to ensure adherence to study protocols.
• Monitor data quality and accuracy in Care Management Platform.
• Facilitate collaboration among service providers and knowledge exchange between practitioners and research partners.
• Perform other duties as assigned by supervisors.

Job description

Requirements:

• Holder of a Registered Nurse Certificate and a valid Practising Certificate issued by the Nursing Council of Hong Kong;
• English Language (Syllabus B) and Chinese Language at Grade E or above in HKCEE/Level 2 in HKDSE;
• Minimum 2 years of clinical experience, with supervisory and project management experience preferred;
• Possession of electronic certificates for the Child Protection Online Course (Module 1 and Module 2) is required;
• Experience in elderly care, community healthcare or integrated care services is a must;
• Knowledge of research methodologies, health informatics and quality management systems is preferable;
• Demonstrated digital literacy and data management capability, with experience in using client information systems, case management platforms, or healthcare databases will be an advantage;
• Strong leadership, communication, stakeholder engagement, and report-writing skills.

Job Duties:

• Lead the planning, implementation, monitoring, and evaluation of a Project supporting older people to develop and/or maintain intrinsic capability;
• Supervise project staff and coordinate multidisciplinary teams across elderly service units;
• Provide clinical oversight for screening, assessment, intervention, and care management processes to ensure compliance with WHO ICOPE protocols and evidence-based practices;
• Monitor the quality and consistency of screenings and assessments conducted by trained volunteers and other social and health care professionals;
• Act as the primary contact person with the research team at a local university to coordinate research implementation and ensure adherence to approved research methodology and study protocols;
• Monitor data quality, completeness, and accuracy within the Care Management Platform;
• Maintain effective communication with participating elderly service units and community healthcare providers, and facilitate collaboration among multidisciplinary service providers;
• Facilitate knowledge exchange between practitioners and research partners;
• Perform other related duties as assigned by supervisors.

Working Hours:

Average working hours of 44 hours per week

Working Location:

Kwun Tong District with outreach community work required
