Job summary

The Lok Sin Tong Benevolent Society, Kowloon invites qualified nurses to join our team. You will conduct health risk assessments and physical evaluations for elderly clients, document findings, and provide targeted health education focused on disease prevention and healthy aging.

As a case manager, you will coordinate referrals to family doctors and specialists, monitor progress, and ensure adherence to care plans within a multidisciplinary team.

Qualifications

• Hold a valid Registered Nurse practicing certificate issued by the Nursing Council of Hong Kong
• Degree in Nursing or equivalent with a minimum of two years of relevant nursing experience
• Experience in elderly health services and community health nursing; good communication skills
• Willingness to pursue qualifications in gerontology nursing

Responsibilities

Entry Requirements

• Basic understanding of common geriatric health issues and willingness to pursue qualifications in gerontology nursing
