Job summary

Haven of Hope Christian Service is seeking a dedicated dietitian to deliver dietetic services for the elderly within home care teams. The role includes home visits, direct assessment, carer education, and coordinating workshops and menu modifications.

You will work as part of a multi-disciplinary team to contribute to care plans and service planning, and provide staff education and coaching. Candidates should hold a BSc in Dietetics with internship, be on the HK Academy of Accredited Dietitians

Qualifications

Bachelor of Science in Dietetics with internship and eligibility for HK Academy of Accredited Dietitians registration.
Minimum 2 years post-qualification experience in dietetic service.
Strong communication and presentation skills.
Proactive, mature and independent; good computer literacy.

Responsibilities

Provide dietetic services to elderly of the home care service teams, including conducting home visits, direct assessment, carer education as well as coordinating workshops, programs and menu modification
As an active member of the multi-disciplinary team, provide professional inputs in care plan formulating
Provide professional inputs in service planning
Provide education & coaching to the staff
