The Supreme HR Advisory Pte Ltd is seeking a Nursing Healthcare Coordinator to coordinate ICS referrals, arrange start of care services, and manage caregiver assignments.
Based at Tagore Lane with shuttle service to AMK MRT, work hours Monday to Friday 9am-6pm, coordinating with hospitals, clients, and families to ensure seamless service delivery.
Responsibilities
- Coordinate and manage ICS referrals from hospitals
- Contact clients and families to arrange the start of care services
- Coordinate Nursing Aide/Caregiver assignments and schedules
- Monitor daily attendance and ongoing client services
- Arrange replacement staff when necessary
- Communicate regularly with clients, families, hospitals and the ICS team
- Monitor clients' service periods and upcoming discharge dates
