Job summary

Bupa Global, the international health insurance division of Bupa, is seeking a Care Coordinator in Hong Kong to improve patient outcomes along the care journey. You will review care needs, direct patients to contracted providers, manage pre-admission and discharge activities, and work with hospitals to ensure timely and efficient care.

Strong analytical and communication skills are essential, with preferred bed management and discharge planning experience.

Qualifications

• Clinical background preferred.
• Bed management and discharge planning experience preferred.
• Strong analytical and decision-making skills.
• Excellent interpersonal and communication skills.
• Good IT skills; comfortable with clinical software.

Responsibilities

• Review care needs and triage patient cases to contracted providers.
• Coordinate pre-admission and discharge processes with hospitals.
• Update patient management systems with clinical information.
• Educate customers and families about care pathways and services.
• Participate in case review meetings and coordinate with care teams.
• Monitor care delivery and escalate issues affecting plan of care.
