A healthcare provider in the UK is looking for a well-qualified GP Nurse to join their team in Conisbrough. The role requires flexibility and dedication to patient care, covering various areas of GP nursing. Ideal candidates will have experience in chronic disease management and excellent communication skills while working autonomously and as part of a team. Good IT skills and a strong commitment to continuous improvement are essential. This is an exciting opportunity in a supportive and friendly work environment.
We are looking for a friendly, well qualified, driven and experienced GP nurse (prescriber would be a bonus) who can slot into our close knit team and help out in all areas of nursing.
We need someone who will muck in when needed, is driven by change and improvement and won't shy away from the busy workload and making house calls. Our practice team work hard and go above and beyond for our patients which bring great rewards in the form of high praise from colleagues and patients, and a great place to work.
We would like someone to work approximately 30 hours per week. Must be available to work from Monday to Friday, with some early evening shifts.
There is the opportunity to work extra hours in our practice as contribution to the extended access hub either during the week or at weekends (this is not part of the contract and is carried out on a voluntary basis)
We would require someone to be able to cover all areas of GP nursing, from wound care and cytology to long term condition reviews and management.
To be able to work as part of a multi-disciplinary team across the practice and network. The successful candidate will have the ability to be flexible to our practice needs and be able to manage patient care in both practice clinics and in the community to help manage our housebound patient population.
We are a medium sized practice with a patient population of approximately 12,000. We are owned by 6 partners, who have a drive and vision to be innovative and forward thinking in their approach to patient care and access. Our main aim is to invest in our practice and community so that we can give the best possible care to all.
Supporting our partners are 2 HCA's, 2 nurses, and 2 ANP's, a paramedic, a Physician associate, 2 clinical pharmacists.
We are a training practice with between 10 and 12 registrar trainees at any one time.
The clinical team is in turn supported by a large reception team, a prescription clerk, 2 secretaries and a variety of other admin staff.
We are all very friendly, yet driven to provide the best care possible, and are continuously looking at ways in which we can improve our services.

Induction
At CGP,you will be required to complete the induction programme and the practicemanagement team will support you throughout the process.
Learningand development
Theeffective use of training and development is fundamental in ensuring that allstaff are equipped with the appropriate skills, knowledge, attitude andcompetences to perform their role. All staff will be required to partake in,and complete mandatory training as directed by the partners. It is an expectation for this post holderto also assess their own learning needs and undertake learning as appropriate.
Allstaff are to recognise the significance of collaborative working andunderstand their own role and scope and identify how this may develop overtime. Staff are to prioritise their own workload and ensure effectivetime-management strategies are embedded within the culture of the team.
Managinginformation
Allstaff should use technology and appropriate software as an aid to managementin the planning, implementation and monitoring of care and presenting andcommunicating information.
Servicedelivery
Staffwill be given detailed information during the induction process regardingpolicy and procedure.
Thepost holder must adhere to the information contained within the organisationspolicies and regional directives, ensuring protocols are always adhered to.
Security
Thesecurity of the organisation is the responsibility of all personnel. The postholder must ensure they always remain vigilant and report any suspiciousactivity immediately to their line manager.
Professionalconduct
Allstaff are required to dress appropriately for their role and will be issued with 2 new tunics or dresses every couple of years.
Leave
Allpersonnel are entitled to take leave allowing for a 6 week notice period for approval.
Public holidays will be calculated on apro-rated basis dependent on the number of hours worked.
