We are seeking an experienced and motivated Band 6 Physiotherapist to join our supportive and dynamic Crisis Response Team in Trafford.
The Crisis Team is part of the South Manchester Community Response Service, which also includes Discharge to Assess and the Community IV team. This is an exciting opportunity to be part of an integrated, multi-disciplinary, and multi-agency team providing rapid assessment and interventions to our local population. Our progressive working environment manages daily crises, offering a 7-day, extended hours, urgent response to referrals from community and acute colleagues, as well as directly from the North West Ambulance Service (NWAS). As a team member, you will collaborate closely with highly trained advanced clinical practitioners and the wider multidisciplinary team, supporting service users to be treated in the community.
Rapid assessment and community intervention from the Crisis Response team help support adults for up to 48 hours with health or social care needs at home, preventing unnecessary A&E attendances. Your clinical skills will contribute to enabling people to remain safely and supported in their own homes.
