What you will do:

• Support individuals who have been discharged from hospital in regaining their independence, enabling them to remain comfortably in their own homes.
• Visit patients at their residences to perform in-depth needs assessments, implement treatments, and offer valuable advice for a smooth transition back into the community.
• Collaborate closely with a multidisciplinary team, which includes Reablement Support Workers and other healthcare professionals, to design and implement personalised recovery plans for each patient.

Requirements of the Occupational Therapist:

• To succeed in this role, you must hold a degree or equivalent qualification in Occupational Therapy and be registered with the HCPC.
• Recent UK and NHS experience as an Occupational Therapist is essential to meet the demands and expectations of this position.
