No Limits is a Southampton-based charity delivering youth services. We are seeking a Youth Worker to join the Isle of Wight team as a community-based social prescriber, supporting children and young people discharged from the emergency department to access community services.

You will build relationships, provide information, signpost, and collaborate with hospital staff to promote wellbeing and prevent crises, with a strong emphasis on safeguarding and DEI values.

Formación

• Experience working with children and young people.
• Minimum Level 3 youth work qualification or equivalent.
• Excellent communication and organisational skills.
• Able to travel to community locations and hospital sites.

Responsabilidades

• Engage and build relationships with children and young people in crisis to avoid hospital admissions.
• Provide ongoing information, signposting and referrals to community-based interventions.
• Work within the YIACS model to address wider wellbeing issues.
• Use tools to measure wellbeing and evidence impact of interventions.
• Support hospital staff and develop relationships with departments to promote the service.
