Job summary

Waythrough is seeking a Complex Needs Practitioner to join the Transitions Team in Birmingham, providing person-centred, strengths-based support to young people aged 17–25 with complex needs related to substance misuse and mental health. You will work across community and multi-agency settings, building relationships and tailoring interventions.

The role focuses on identifying goals, reducing risk, and developing resilience, with duties across health, social care, education and criminal justice

Qualifications

• Experience within health or social care
• Experience working with young people and supporting those with mental ill health and/or substance misuse
• Empathy, energy and belief in young people0s potential

Responsibilities

• Support young people aged 1725 with complex needs, including substance misuse, mental health and housing instability.
• Build trusting relationships and provide tailored interventions across the community and multi-agency settings.
• Respond to referrals from health, social care, education and criminal justice and promote resilience and independence.
