Job summary

Horizon Care Ltd in Wiltshire is seeking a Youth Support Worker to assist young people in transitioning to independent living. You will cover Trowbridge and Chippenham, providing emotional and practical support, safeguarding, and helping access education, training, and employment.

Qualifications

• Experience working with vulnerable young people, ideally care leavers.
• Strong safeguarding knowledge and assessment skills.
• Ability to be resilient and empathetic when working with our young people.
• Excellent communication and problem-solving abilities.

Responsibilities

• Provide emotional and practical support, including out-of-hours on-call.
• Develop and review support plans, ensuring safeguarding and risk management.
• Help young people access education, training, and employment.
• Manage properties and liaise with other professionals.
• Support our young people to develop independent living skills to prepare them to live fully independently.
