Job summary

Horizon Care Ltd in Burnley is seeking a dedicated Youth Support Worker to help young people transition to independent living. You will provide emotional and practical support, including out-of-hours on-call, and work to develop plans that safeguard and guide each young person.

You will assist with access to education, training and employment, manage properties, and collaborate with professionals to support independent living skills.

Qualifications

• Experience working with vulnerable young people, ideally care leavers.
• Strong safeguarding knowledge and assessment skills.
• Ability to be resilient and empathetic when working with our young people.
• Excellent communication and problem-solving abilities.

Responsibilities

As a Youth Support Worker you will be helping young people transition to independent living, duties include:

• Provide emotional and practical support , including out-of-hours on-call.
• Develop and review support plans, ensuring safeguarding and risk management.
• Help young people access education, training, and employment.
• Manage properties and liaise with other professionals.
• Support our young people to develop independent living skills to prepare them to live fully independently .

What we’re looking for from you:

• Experience working with vulnerable young people, ideally care leavers.
• Strong safeguarding knowledge and assessment skills.

• Flexibility to be able to work shifts that include evenings, weekends and sleep ins.
• A driving license and access to a vehicle.
