Job summary

Mersey Care NHS Foundation Trust is recruiting a Band 6 Mental Health Transition Practitioner to support 18-25s moving into the CMHT/Recovery Team and connect them to their chosen pathway and interventions. The post holder will engage the young adult, carer and referrer, carry out biopsychosocial assessments and act as keyworker to guide transition in line with NICE standards.

Qualifications

• Degree-level training or equivalent and current UK professional registration.
• Experience in mental health care and care coordination.
• Experience providing transition support for 18-25s.
• Ability to work within MDT and across CYPS/AMHS and wider system.

Responsibilities

• Act as keyworker for 18-25 transitioning into CMHT/Recovery Team.
• Conduct biopsychosocial assessments and support transition planning.
• Collaborate with CYP services and wider system to ensure seamless transition.
• Monitor transition standards and contribute to service improvements.
