Mersey Care NHS Foundation Trust is recruiting a Band 6 Mental Health Transition Practitioner to support young adults aged 18-25 transitioning from CYP to Adult CMHT/Recovery teams. The role requires engaging the young adult, carer and referrer to inform treatment choices and ensure smooth transfer.

The post holder will perform biopsychosocial assessments, coordinate care, and act as keyworker while collaborating with CYP services and wider system to deliver a seamless transition in line with

Qualifications

• Degree level training or equivalent in a related field.
• Registered professional with NMC/SWE or equivalent.
• Post-registration knowledge and experience; autonomous practice.
• Evidence of CPD and ongoing development; ideally master’s level.
• Experience in transition or related mental health services desirable.

Responsabilités

• Oversee NICE transition statements and ensure timely transitions.
• Lead planning with CYP and AMHS to coordinate care.
• Act as keyworker during transition period for 18–25s.
• Collaborate with CYP Services and wider system for seamless care.
• Assess biopsychosocial needs and support transition into recovery cmht.
