Job summary

Lancashire Care Foundation Trust in the North West invites applications for a Community Mental Health Practitioner in Sector Teams. The post involves carrying out mental health assessments and delivering clinically appropriate interventions for children and young people referred to the service.

You will provide support and consultation to families and carers, work within a multi-disciplinary team and in hospital and community settings, and contribute to supervision and continuous service

Qualifications

• Undertake mental health assessments for children and young people referred to the service.
• Provide evidence-based clinical interventions informed by assessed needs.
• Offer support, advice and consultation to families/carers and other professionals.

Responsibilities

• Deliver mental health assessments for children and young people.
• Provide appropriate, evidence-based clinical interventions.
• Work within a multi-disciplinary team and as a sole practitioner when required.
• Ensure work is aligned with best practice and guidelines.
