South West Yorkshire Partnership NHS Foundation Trust is recruiting an experienced Mental Health Practitioner for the Wakefield CAMHS Enhanced Outreach Team. You will hold a caseload of young people with complex mental health needs and work within a busy multidisciplinary team to deliver evidence-based interventions.

You will attend MDT meetings, liaise with Social Care Direct, conduct risk assessments, keep documentation up to date, and support families and carers.

Qualifications

• Experienced Mental Health Practitioner with registration.
• Experience working with children in care and their families.
• Ability to work autonomously within supervision framework.

Responsabilités

• Hold a caseload of young people with complex mental health concerns and provide therapeutic intervention.
• Attend MDT meetings.
• Liaise with other agencies, such as Social Care Direct, regularly.
• Ensure documentation and paperwork is up to date and accurate.
• Conduct risk assessments and support young people, carers/parents to manage identified risks.
• Offer therapeutic intervention to young people on your caseload.
• Support with the duty worker rota (within working hours).
