Job summary

Brook Street in Slough is seeking an Outreach Worker to support young people aged 16-25. You will deliver person-centred outreach in the community, helping with housing, education, employment, health and wellbeing.

This part-time role offers flexible hours (10/15/20 hours per week) with sessions planned between 9am-5pm, Monday to Friday, and across weekends if needed. Driving licence and travel across SL1-SL6 are essential.

Qualifications

• Experience working with young people, especially those facing disadvantage.
• Ability to build trusting, professional relationships.
• Strong communication and organised approach to caseload management.

Responsibilities

• Deliver tailored, one-to-one outreach support to young people in homes and community settings.
• Help develop independent living skills: budgeting, cooking, tenancy management and safe home environment.
• Assist them to access education, training, employment and volunteering opportunities.
• Provide guidance on housing options, benefits and relevant services.
• Signpost to multi-agency services under guidance of the service manager.
• Develop and review person-centred support plans with clear goals.
• Promote positive mental health and emotional wellbeing.
• Advocate with external agencies and professionals.
• Maintain accurate case records in line with policies.
• Safeguard and promote welfare and participate in supervision and development.
