Job summary

Codi Group in Ammanford (Carmarthenshire) is recruiting a Part-time Project Worker (Complex Needs) to support the Manager with young person goal plans and to advise on re-housing options within a Young Persons Supported Accommodation scheme.

You will liaise with Social Services, Benefits Agencies, Housing Departments and the Resettlement Team, make referrals to other agencies, and promote mental health recovery and access to substance misuse treatment where appropriate.

Responsibilities

• Support the Manager with facilitating Young Person goal plans.
• Advise on re-housing options for young people within a supported accommodation scheme.
• Make referrals to other agencies for temporary and permanent accommodation with the resettlement team.
• Liaise with Social Services, Benefits Agencies, Housing Department, Resettlement Worker, and Health & Safety on client matters.
• Promote mental health recovery and seek treatment and recovery for substance misuse where applicable.

Job description

A Part-time (32hrs) Project Worker (Complex Needs) in a Young Persons Supported Accommodation scheme in the Ammanford area of Carmarthenshire.

As a Complex Needs Project Worker your role will be to support the Manager with facilitating Young Person goal plans and will work extensively with them to advise on re-housing options. You will make referrals to other agencies in relation to temporary and permanent accommodation in conjunction with our resettlement team.

You will liaise with Social Services, Benefits Agencies, Housing Department, Resettlement Worker, Health and Safety and other relevant agencies, on matters affecting prospective and current clients of the service. Part of your role will be to promote mental health recovery and seek treatment and recovery for substance misuse where applicable.
