Job description

Job Title: Young Person's Practitioner (Risk and Resilience)

Type: Permanent, full-time
Salary: Up to £29,000 per annum
Requirements: Access to a car and a full driving licence

This role offers an excellent opportunity to make a meaningful difference in the lives of children and young people (C&YP) at risk of child exploitation, substance misuse, or going missing from home. You will provide tailored support and guidance, contributing to high standards of care, quality, and positive outcomes, helping service users reach their potential through consistent participation.

Main Duties:
  • Providing tailored one-to-one support, guidance, and interventions.
  • Assessing needs, developing support plans, and informing risk reduction strategies, including facilitating access to specialist services.
  • Ensuring high-quality referrals, support, care, and risk management with smooth transitions.
  • Developing sustainable joint service work with agencies, local authorities, and children’s social care.
  • Maintaining accurate data records, tracking outcomes, monitoring KPIs, and assisting in feedback report production.
  • Providing training, information, and presentations to diverse stakeholders, with opportunities for skill development in training provision.

This role involves working with partner agencies and commissioners, offering opportunities to expand your network and knowledge within the Children’s Social Care Sector. We support your professional development and encourage visits to national services offered by Catch22.

Candidate Experience:

Qualifications:

Additional Information:

Salary: Up to £29,000 depending on experience
Hours: 37 hours/week
Location: Civic Centre, Morden

We have two positions; one is open to male applicants to ensure accessible support for young people. The role requires a car and full driving licence. We promote flexible working and community-based service delivery, with options for remote work where feasible.
