Job Description

The role of Substance Misuse Practitioner is an excellent opportunity to join a highly communicative team of professionals, who are actively working as part of a multi-agency team to support and empower at-risk children and young people. This role will provide appropriate specialist or targeted interventions to a caseload of young people, who are referred by means of effective assessment, care planning, and risk management.

*Access to a car, and a full driving licence, is a requirement of this role.

Main Duties and Responsibilities

• Develop, monitor, evaluate, and report on care plans, YPORS risk assessments, and outcomes throughout the young person’s treatment journey, providing appropriate care and support.
• Provide family interventions, support parents and carers of service users, and those wishing to access our services.
• Promote and support service user involvement through facilitation of service user groups and activities, and support volunteer participation within the service.
• Work in partnership with other agencies, promoting effective dialogue, referral, and care pathways between agencies, families, and schools.
• Collaborate with criminal justice services to support young people addressing their substance misuse problems and reducing re-offending risk.
• Deliver harm minimisation and drugs education activities, supporting individuals and groups in schools and other settings.
