Job summary

Sheffield City Council is seeking an Inclusion Officer (Young Carers Assessor) to work directly with children and young people with caring responsibilities. You will identify needs, undertake assessments and ensure the young person’s voice stays central in every decision.

You will collaborate with schools, health professionals, social care and voluntary partners to plan support, transitions and improved outcomes for Sheffield's remarkable young carers.

Qualifications

• Experience working with children, young people or families.
• Understanding challenges faced by young carers.
• Assessment, engagement and person-centred planning skills.
• Strong communication, organisational and recording skills.
• Ability to work independently within a supportive team.

Responsibilities

• Identify needs, strengths and aspirations of young carers.
• Undertake assessments that centre the young person’s voice.
• Build trusted relationships with families and partner agencies.
• Help plan support, transitions and improve outcomes.
