Carers Trust Crossroads West Wales is seeking a Young Carers Transition Officer to support young adult carers aged 16 to 25 in balancing their caring role with wellbeing, independence and future goals.
You will provide one-to-one support, develop personalised support plans, and deliver activities and respite opportunities that help young adult carers build confidence, resilience and connections. Successful applicants may be asked to travel across Carmarthenshire.
