Job summary

Forward Trust is seeking a dedicated Recovery Programme Practitioner (Y Bont) to support men in custody at HMP Swansea. You will deliver 1:1 and group interventions, manage caseloads, and collaborate with prison staff, healthcare, and community partners to enable sustained recovery and smoother community transition.

The role is based primarily at HMP Swansea (35 hours/week) with occasional travel across South Wales; prior experience in addiction recovery and a collaborative approach are

Qualifications

• Experience supporting people affected by substance misuse, recovery or criminal justice services.
• Strong communication and group facilitation skills.
• Experience managing caseloads and keeping accurate records.

Responsibilities

• Deliver structured 1:1 and group interventions focused on substance misuse recovery.
• Co‑facilitate the Y Bont Programme across multiple cohorts each year.
• Manage a caseload, assessments, care plans and progress reviews.
• Use trauma‑informed, harm reduction and abstinence‑based approaches tailored to individual needs.
• Work closely with prison staff, healthcare, probation, housing, employment and community partners.
• Support release planning and continuity of care into the community.
• Monitor outcomes, maintain accurate records and contribute to service targets.
• Help organise graduation ceremonies and family engagement events.
