Job summary

Wiltshire Council in Salisbury is recruiting Reablement Support Workers to help clients recover at home after hospital discharge. The role involves working with occupational therapists to co-develop personalised reablement plans, travel to client visits, and provide compassionate support across a 32-hour week.

Applicants should have or be prepared to pursue NVQ apprenticeships, hold a valid UK driving licence and be comfortable with a rota pattern across 7am-10pm, 365 days a year.

Our Reablement Support Workers provide vital assistance to individuals in their homes post-hospital discharge. They play a pivotal role in helping customers achieve their goals, working closely with dedicated occupational therapists to co-develop personalised reablement plans.

We are seeking individuals with a genuine passion for helping others who are available on a rota basis between 7am to 10pm 365 days a year. This role involves travel for customer visits. You will need a valid UK driving licence and access to a vehicle, unless other suitable transport or a reasonable adjustment is agreed.
