Job summary

Wiltshire Council is seeking Reablement Support Workers to help individuals at home after hospital discharge. You will support customers to achieve goals, working with occupational therapists to co-develop personalised reablement plans.

Role requires flexibility to work on a rota between 7am and 10pm, 365 days a year, and travel to client visits. A valid UK driving licence and access to a vehicle are essential, with hybrid office/community work model.

Salary:£13.47 - £13.90 per hour (Plus 10% unsocial hours plussage)

Hours per week:30, 24 & 27 hours

Our Reablement Support Workers provide vital assistance to individuals in their homes post-hospital discharge. They play a pivotal role in helping customers achieve their goals, working closely with dedicated occupational therapists to co-develop personalised reablement plans.

We are seeking individuals with a genuine passion for helping others who are available on a rota basis between 7am to 10pm 365 days a year.
