Kent Community Health NHS Foundation Trust is seeking a Wound Clinic Practitioner to deliver specialist wound care to housebound patients across East Kent, in clinic settings and in patients' homes when required. You will work within a supportive multidisciplinary team aligned to Primary Care Networks, with access to Patient Transport.

You will deliver evidence-based interventions and holistic assessments that promote healing, independence and improved quality of life.

Qualificações

• Deliver evidence-based interventions and holistic assessments that promote healing, independence and improved quality of life.
• Manage a varied caseload with assessment, planning, implementation and evaluation of personalised care for patients with complex wound care needs.
• Change wound dressings, catheter care, holistic assessments and clinical interventions within scope.
• Supervision and development of junior colleagues, Healthcare Assistants and students.

Responsabilidades

• Assess and plan personalised wound care for housebound patients.
• Deliver evidence-based wound interventions and monitor healing.
• Collaborate with multidisciplinary teams to coordinate care and referrals.
