Job summary

Carers’ Resource in Bradford is seeking a Locality Worker – Family Coordinator Specialism to support unpaid carers across the district. The role focuses on a whole-family, relationship-based approach, carrying out assessments and coordinating services to help parent and young carers.

Based at the Shipley office, 30 hours per week, with a salary of £27K–£28,250 per year. You will work with Neighbourhood Connector Teams and external partners to deliver joined-up support.

Qualifications

• Experience coordinating family-based services and support.
• Ability to assess family needs and develop tailored plans.
• Knowledge of safeguarding policies and professional boundaries.

Responsibilities

• Identify and support whole families, linking parent, young and elder carers.
• Carry out carer assessments and develop tailored support plans.
• Coordinate across internal teams and external partners for joined-up delivery.
• Champion transitions for young carers moving into adult services.
• Maintain accurate records and uphold safeguarding practices.
