Job summary

Solent Mind is seeking a Wellbeing Assessor to undertake assessments of patients and determine the most suitable support within the Primary Care Mental Health Service and partnering organisations.

You will need experience with vulnerable people in a mental health setting, plus strong communication, IT and teamwork skills to succeed.

• Salary: £13,452.50 to £14,780 per annum (£26,905 to £29,560 full time equivalent per annum) - Solent Mind Band C. In line with organisational policy, new appointments will ordinarily be made at the starting salary point of the advertised grade.
• Hours: 18.5 per week
• Location: Eastleigh PCN ( GP Surgeries in Hamble and Hedge End) and some home working
