Job summary

Derbyshire Community Health Services NHS Foundation Trust in Derbyshire is seeking a Weight Management Adviser to join the Specialist Tier 3 Weight Management Service. Qualifications

• Level 3 exercise referral qualification (or equivalent) such as YMCA FIT Diploma in exercise referral or Wright Foundation exercise referral or willing to train.
• Level 3 nutrition and weight management qualification (or equivalent) or willing to train
• Training in motivational interviewing/behaviour change/counselling or relevant experience

Responsibilities

• Deliver evidence-based weight management assessments, education, advice and ongoing support to adults with complex obesity.
• Manage a caseload of patients receiving specialist Tier 3 weight management support, providing personalised coaching and behaviour change interventions.
• Work collaboratively with the wider multidisciplinary team, ensuring patients access specialist dietetic, psychological and medical support when appropriate.
• Facilitate and support both face-to-face and virtual group education sessions.
• Support patients to set realistic goals, monitor progress and identify barriers to achieving sustainable lifestyle change.
• Collect, record and interpret outcome measures in line with service requirements and quality standards.
• Undertake baseline assessments of physical health, medications, obesity-related conditions and lifestyle factors in accordance with service protocols.
• Contribute to service development, quality improvement initiatives and the creation of patient resources.
• Maintain accurate clinical records and work in accordance with professional, organisational and information governance standards.
