Job summary

Swindon Borough Council is looking for a Support Worker in the Reablement Team for weekend shifts to support residents to regain independence at home. You will deliver short-term personal care and outcome-focused support within the community as part of a multidisciplinary team.

Successful candidates will have experience with vulnerable adults, be able to travel across the borough, and hold a valid driving licence. DBS check required; salary £26,824 per annum with pay award applied.

Qualifications

• Experience caring for vulnerable adults with complex needs.
• Experience in community or care home settings.
• Must be fluent in English.

Responsibilities

• Provide personal care and reablement support to service users in their homes and community.
• Assess service users’ needs and monitor against care plan objectives and outcomes.
• Maintain accurate records and liaise with other professionals.
