Job summary

Swindon Borough Council is seeking a Support Worker for the Reablement Team to deliver short-term, outcome-focused support in clients’ homes and local communities. The role emphasises dignity, independence and person-centred care within the Reablement Team.

You will assess needs, monitor progress, liaise with professionals and support individuals to regain skills while maintaining accurate records. A full driving licence and access to a vehicle are required due to travel across the Borough.

Qualifications

• Must be fluent in English to support customer-facing roles.
• Experience in caring for vulnerable adults is preferred.
• Ability to travel throughout the Borough and work in service users' homes.
• Willingness to undergo enhanced DBS checks.

Responsibilities

• Provide personal care and reablement support to service users in their own homes and community.
• Assess service users' needs and monitor progress against care plan aims and outcomes.
• Liaise with other professionals to coordinate care.
• Assist with tasks such as bills, shopping and prescriptions when needed.
• Promote positive links with local community agencies and groups.
• Maintain accurate records and report accidents or concerns.
