Job summary

HCRG Care Group is inviting applications for an Associate Practitioner on Sapphire Ward at Gravesham Community Hospital. You will work under a Registered Nurse, delivering patient-centered rehabilitation and supporting a multidisciplinary team to maximise independence.

The role involves dressing, glucose monitoring, and contributing to care planning, with emphasis on safe, compassionate practice and ongoing development within a supportive environment.

Qualifications

• Current NMC registration.
• Strong communication and interpersonal skills.
• Ability to work independently as part of a multidisciplinary team.
• Good organizational skills and ability to manage competing priorities.
• Comfort with IT systems and EMIS.

Responsibilities

• Deliver safe, compassionate patient care under supervision of a Registered Nurse.
• Undertake delegated clinical interventions (dressings, glucose monitoring).
• Support assessment, planning, implementation and evaluation of care.
• Monitor progress and elevate changes or concerns.
• Maintain accurate clinical documentation using EMIS.
