Job summary

Gravesham Community Hospital is seeking an Associate Practitioner to join Sapphire Ward, delivering high-quality rehabilitation care under the supervision of a Registered Nurse. You'll work within a multidisciplinary team to support patients to regain independence and achieve their goals, developing core clinical skills and providing patient-centered care.

The role requires NMC registration, experience in health or social care, and strong communication.

Qualifications

  • Current NMC registration.

Responsibilities

  • Deliver safe, compassionate patient care in line with policies and standards.
  • Undertake delegated clinical procedures such as dressings and glucose monitoring.
  • Support assessment, planning, implementation and evaluation of patient care under supervision.
  • Monitor patient progress and promptly report concerns.
  • Maintain accurate clinical documentation using EMIS and other systems.
  • Support rehabilitation programmes to promote independence and recovery.
  • Assist with ward administration, stock checks and equipment maintenance.
  • Participate in MDT meetings, audits and quality improvement initiatives.
  • Contribute to induction and training of new colleagues and students.
  • Uphold infection control, safeguarding and information governance standards.

About the Role

We have an exciting opportunity for an Associate Practitioner to join our friendly and supportive team on Sapphire Ward at Gravesham Community Hospital. Working within a rehabilitation setting, you'll play a key role in supporting patients to achieve their goals and maximize their independence. Working under the supervision of a Registered Nurse and as part of a multidisciplinary team, you'll deliver high-quality, person-centered care while developing and maintaining a broad range of clinical skills. This is an excellent opportunity for an NMC-registered practitioner who enjoys working closely with patients and colleagues to make a meaningful difference to patient outcomes.

Main Responsibilities
  • Deliver safe, effective and compassionate patient
    care in line with organizational policies, procedures and professional standards
  • Undertake delegated clinical interventions, including dressings, glucose monitoring and other agreed nursing procedures
  • Support the assessment, planning, implementation and evaluation of patient care under the direction of a Registered Nurse
  • Monitor patients' progress and promptly escape any changes or concerns
  • Maintain accurate and timely clinical documentation using electronic patient record systems, including EMIS
  • Support rehabilitation programmes that promote independence and recovery
  • Assist with ward administration duties, stock ordering and equipment checks
  • Participate in multidisciplinary team meetings, audits and quality improvement initiatives
  • Support the induction and development of new colleagues and students
  • Maintain high standards of infection prevention, safeguarding, information governance and health and safety practices
  • Contribute to creating a safe, caring and positive environment
    for patients, visitors and colleagues

About You

We're looking for a caring, motivated and skilled practitioner who is passionate about providing outstanding patient care.

Essential Requirements
  • Current NMC registration
  • Previous experience working within a health or social care environment
  • Strong communication and interpersonal skills
  • Ability to work independently and effectively as part of a multidisciplinary team
  • Good organizational skills and the ability to manage competing priorities
  • A compassionate, patient-focused approach to care
  • Confidence using IT systems and willingness to learn new electronic patient record systems, including EMIS
  • Ability to assess patient needs and deliver care within your scope of practice under the direction of a Registered Nurse
  • Flexibility to work across a seven-day rota
  • Commitment to ongoing personal and professional development
Desirable

Previous experience within a rehabilitation, community hospital or intermediate care setting Competency or experience in: Venepuncture Catheter care Stoma care Wound management Experience contributing to audits, quality improvement initiatives or service development activities
