Bradford Teaching Hospitals Foundation Trust is seeking a capable leader to provide clinical leadership and line management to the Virtual Services Team. The role focuses on enabling care at home, reducing hospital stays and avoiding admissions while ensuring a supportive, educational environment for staff.

The post holder will drive safe, effective virtual care pathways and work across organisations, including social services and Primary Care Teams, to support discharge and care planning.

Formación

• Significant post-registration experience in an acute trust or community setting.
• Experience of leading and managing change in a clinical setting.
• Experience in policy development and working across organisations.

Responsabilidades

• Coordinate and manage the Virtual Services team and ensure good working relationships with other agencies.
• Use high level communication skills to provide and receive complex information.
• Maintain a professional attitude and develop staff to high professional standards.
• Provide advice and information to staff ensuring consistent high standard of communication.
• Work across organisations with social services and Primary Care Teams to support safe community-based care.
