Bradford Teaching Hospitals Foundation Trust is seeking a capable leader to provide clinical leadership and line management to the Virtual Services Team. The role focuses on enabling care at home, reducing hospital stays and avoiding admissions while ensuring a supportive, educational environment for staff.
The post holder will drive safe, effective virtual care pathways and work across organisations, including social services and Primary Care Teams, to support discharge and care planning.

The Virtual Ward is an innovative service and the aim to is support people to receive care, support and interventions within their own home, this improves the quality of care for the individuals, reduces the length of time people stay in hospital, reduces the need for people to be admitted to hospital and ensure beds are available to treat the acute admissions.
The successful applicant will provide clinical leadership and line management to the Virtual Services Team. The aim of Virtual Ward is to support people to receive care, support and interventions they need in their own home which will lead to improved quality of life and outcomes for people as well as reducing hospital length of stay and unavoidable hospital admissions. They are expected to lead by example, motivate and empower team members to achieve their potential and provide a visible, accessible and authoritative presence in the Virtual Services Team to whom staff, patients and their families can turn to for assistance advice and support in relation to their hospital discharge and care needs.
