Job summary

SECAmb seeks a Virtual Care Clinician (Nurse/Paramedic) Band 6 to join the Integrated Care team in Crawley or Medway. Full- and part-time roles with office-based work at NHS contact centres. The salary range is £39,959–£48,111 per annum.

Recruitment is for a frontline virtual care service to deliver remote assessments, care planning and timely referrals. Locations include Medway Make Ready Centre (East) or Crawley HQ (West).

Qualifications

• Registered Paramedic (HCPC) or Registered Nurse - Adult (NMC) with current professional registration and 2 years post-registration experience.
• Experience in urgent, emergency, unscheduled or virtual care is desirable.

Responsibilities

• Undertake autonomous virtual clinical assessments for patients accessing 999, NHS 111 and other referral pathways.
• Use clinical judgement and evidence-based practice to assess and manage urgent and emergency healthcare needs.
• Develop person-centred care plans and deliver self-care advice, safety-netting and referrals.
• Support access to community, primary care, mental health and urgent care services.
• Provide clinical leadership and advice to operational and clinical colleagues.
• Identify and manage risk, safeguarding concerns and patient deterioration.
• Contribute to quality improvement, audit and service development projects.
• Support the development of innovative virtual models of care and growth of SECAmb's Virtual Care service.
• Coach, mentor and support colleagues as the service evolves.
