Job description

Richmond Response and Reablement Team (RRRT) - Healthcare Professional

RRRT is recruiting a passionate registered healthcare professional, regardless of nursing, paramedic, or therapy background, with assessment skills and a keen interest in preventing unnecessary hospital admissions.

RRRT is a seven-day integrated health and social care service for adults, mainly older people, in partnership with HRCH and the London Borough of Richmond upon Thames Council.

This development role offers support from the multidisciplinary team to develop skills to an advanced practice level. We support diverse backgrounds and are committed to staff development.

The post involves using advanced assessment skills to conduct thorough clinical assessments, diagnose, and treat patients within 2 hours of referral in their homes or care homes. The goal is to prevent clinical deterioration and unnecessary hospital admissions, easing pressures on GPs, ambulance services, and hospitals.

Key Responsibilities
  1. Undertake Rapid Responses within the Community.
  2. Carry out interventions within your scope of practice.
  3. Manage patients with complex long-term conditions as part of the Richmond Response & Rehabilitation Service.
  4. Work autonomously with advanced assessment, decision-making, and problem-solving skills, referring patients to appropriate services.
  5. Enhance outcomes for individuals with long-term conditions, reducing hospital and care admissions.
  6. Collaborate with multidisciplinary teams, including GPs, community health staff, social care, hospitals, voluntary and private sectors, and patients and carers.
Clinical Responsibilities
  1. Assess and provide advice or care to service users.
  2. Use advanced skills to assess, diagnose, plan, initiate/change treatments, and discharge patients, determining if further investigation is needed.
